Memberships give the highest value for patients who schedule consistently. To join, submit the $99 one-time enrollment fee using the secure payment button on this page. Once your payment is received, our office will contact you on the next business day to complete your membership setup. After submitting your enrollment, fill out the T-shirt form so we can set aside your welcome shirt.
